IN-NETWORK WITH ANTHEM BLUE CROSS AS OF DEC 1, 2025 |
Vasectomy is a covered service for most insurances. We do our best to make it simple and transparent.
Below is a list of insurances for which we are in-network and the “allowable.” The allowable is the amount that we have agreed with the insurance as the maximum that can be charged. If you have one of these insurances, we will ask for insurance details when you complete your “intake” form with us. We will then adjust the amount that we are charging you to the allowable. We will collect the allowable from you when you come in for the procedure. Within 3-4 weeks of the procedure, insurance will inform us if they are going to pay us. If they do, then we will refund that amount to you.
| Insurance | Allowable |
|---|---|
| United Healthcare (UHC) | $821.63 |
| UMR | $821.63 |
| Anthem Blue Cross Blue Shield (effective Dec 1, 2025) | $666.15 |
| Rocky Mountain Health Plans | $800 |
| UnitedHealthOne / Golden Rule | $821.63 |
| UnitedHealthcare / All Savers Alternate Funding | $821.63 |
| Surest | $821.63 |
| US Health Group | $821.63 |
| GEHA | $821.63 |
We are out of network for Cigna, Aetna and most insurances that are not on the “in-network” list above .
You pay the cash-pay price of $885 at the time of the appointment. We will give you paperwork to submit to your insurance so that the insurance company”allowed charge” is applied to your deductible. This is usually about $450 for vasectomy. So, you’ll pay a total of $885 and $450 will be applied to your out-of-network deductible. If you have already spent your deductible (or your deductible is less than $450), then you can submit the paperwork to insurance to request reimbursement.
If you HAVE met your deductible, then you will send the paperwork to your insurance company. They should reimburse you the amount that they allow for the procedure, usually $450-$550.
Sorry, we can’t be sure your insurance will work this way. Please speak to your insurance company in advance to verify their policy. Advise them the procedure code is 55250 and that this is being done in an office (not a hospital). The diagnosis code is Z98.52. Tell them our Tax ID number is 87-1513347 and the procedure will be done by Dr Christopher Tonozzi at Go Vasectomy.
We accept CareCredit payments at each of our sites. CareCredit is a line of credit for healthcare expenses. For more information and to apply for a line of credit, see the CareCredit website. We do NOT accept care credit for the deposit that you’ll pay when you make your appointment–just the payment when you come in for the vasectomy.
We do not participate with Medicaid at this time. This includes HealthFirst Colorado, Rocky Mountain Health Plans PRIME or RAE. If you have one of these, we recommend you call a Community Health Center. They accept Medicaid and most of them have providers who perform vasectomies.
