If you’ve landed here, you’ve likely been told you can download a lab order to have a PVSA test done at a Conceptions laboratory in the Denver area.  The button below will open a pdf that you’ll need to print out and complete.  Don’t forget to add the date next to Dr. Tonozzi’s signature.  The locations of the labs are printed on the requisition.  You’ll need to call the location that you’d like to use to schedule an appointment. You’ll make the specimen at the lab.
